spiritual support to individuals and their families nearing the end of life. To
give individuals quality of life when the quantity is limited.
Eligibility: Must be certified by a physician to have a terminal
illness with a prognosis of 6 months or less.
Fees: Covered by Medicare and most insurance companies. No one will be refused
services if they qualify.
Services: Skilled nursing, home health aide, physical therapy,
occupational therapy, speech therapy, medical social worker, chaplain,
volunteers.
Use of Volunteers: To provide direct patient care, assist in the office and
assist with health fairs.

be in progress.

General Purpose: Nutrition for the elderly. To enhance the longevity of the
older persons' life at home.
Eligibility: Must be 60 years of age or older or the spouse of someone
of that age.
Fees: $1.25 is suggested for the meal but no one is denied a meal if they do
not have the money.
Services: Congregate meals at the senior nutrition center, homebound
meals for those who are unable to come to the site, nutrition education, info.
and referral, recreation and socialization at each site, and advocacy for the
senior citizen.
Use of Volunteers: Volunteers are used to make meal deliveries to the
homebound. They also service the disabled that are at the center. The senior
citizens own the building so all maintenance is kept up by volunteers.
